Job Description

Accurate Home Care, LLC is currently seeking qualified candidates for the position of Mental Health Practitioner to work within our Mental Health Division. The Mental Health Practitioner is responsible for providing skills training, and crisis assistance. Responsible for completing necessary clinical documentation including, but not limited to, progress notes while adhering to the Supervision Plan.

Major responsibilities of this position include:

  • Maintains a caseload that consistently meets the necessary weekly billable time
  • Conducts individual, group, and family skills training and completes necessary documentation.
  • Conducts crisis assistance and completes necessary documentation.
  • Ensures that all required paperwork are maintained, filled out professionally, and up to date.
  • Coordinates care with other Accurate Home Care providers regarding shared clients.  Meets regularly to coordinate shared cases.
  • Coordinates care with caregivers, completes necessary documentation needed for care coordination, and attends necessary meetings.
  • Attends supervision with the Clinical Supervisor and fulfills responsibilities outlined in the Supervision Plan. 
  • Seeks out supervision when needed from the Clinical Supervisor/Mental Health Professional.   
  • Maintains continuing education hours for the position and attends required trainings.     

As assigned:

  • Communicates and maintains a current schedule of hours available to work and preferences. 
  • The scheduling for this role is completed by the Scheduling/Team Coordinator and the practitioner adheres to the daily schedule. 
  • Communicates in a professional and timely manner. 

Job Requirements

In order to be considered for this position, the candidate must meet the following criteria:

Education:

(1) Holds a bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university; and


  • (a) has at least 2,000 hours of supervised experience in the delivery of mental health services to clients with mental illness; or§

(1) A bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university and  have at least 2,000 hours of supervised experience in the delivery of mental health services to children with emotional disturbances; or;

(2) Has at least 6,000 hours of supervised experience in the delivery of mental health services; hours worked as a mental health behavioral aide I or II under MN Statute section 256B.0943, subdivision 7, may be included in the 6,000 hours of experience; or

(3) Is a graduate student in one of the behavioral sciences or related fields and is formally assigned by an accredited college or university to an agency or facility for clinical training; or

(4) Holds a master's or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university.

Competencies:

  • Proficient with conducting skills training, crisis assistance, and direction of MHBA and completing necessary documentation.
  • Ability to communicate, demonstrate, and understand mental health terminology, procedures, and requirements.
  • Maintains regulatory requirements, including all state and federal regulations
  • Verbal and written communication, organization, attention to detail, and customer service skills while maintaining confidentiality.
  • Demonstrate empathy and compassion for individuals who are chronically ill or disabled.
  • Skilled in operating a personal computer and in the use of various software applications including, MS Word, Excel, Outlook, PowerPoint, Internet search engines, and electronic health records.
  • Skilled in operating various office equipment such as photocopier, facsimile, calculator, and telephone system.
